This form is for Derry Area High School graduates, Classes 2021 or earlier.

If you are in the Classes 2022 or later, please use the Parchment site, https://www.parchment.com/u/registration/20798/account (link).

If this transcript is for a Common App, DO NOT complete this form. Common App will automatically send an electronic transcript request to your school counselor.

Classes 2021 or earlier, please complete this form for each transcript request.

There are two types of high school transcripts, official and unofficial.

Official transcripts are sent directly to post-secondary schools and delivered in a sealed, signature-stamped envelope or electronically from a school representative. If you are picking up an official transcript and delivering it to a college, the envelope must remain sealed.

Unofficial transcripts can be given directly to you and are for your own personal records or for employment.

Please fill out one form per each transcript requested. Example; if you need one official and one unofficial, please fill out 2 seperate forms.

The Counseling Office will process a high school transcript within 5 business days.
